Introduction

Endovenous Laser Treatment (EVLT) — also called endovenous laser ablation (EVLA) — is the most widely performed minimally invasive treatment for varicose veins globally. At CARE Hospitals, Banjara Hills, I perform EVLT as a day procedure under local anaesthesia with ultrasound guidance. This guide explains exactly what EVLT involves, what patients experience, and what to expect during recovery. For an overview of all treatment options, see: Varicose Veins Treatment in Hyderabad.

💡 How Does EVLT Work?

A fine laser fibre is inserted into the affected vein through a small needle puncture in the skin — no incision is required. The fibre is positioned under continuous ultrasound guidance. Tumescent local anaesthesia (dilute local anaesthetic fluid) is then infiltrated around the vein. The laser is activated as the fibre is slowly withdrawn, delivering precisely controlled heat energy that causes the vein wall to collapse and seal permanently. The closed vein is then gradually absorbed by the body over weeks to months.

📝 Step-by-Step: What Happens During the Procedure

Before the Procedure

A Duplex ultrasound scan is performed to map your veins precisely. You can eat and drink normally beforehand. Wear comfortable, loose clothing. Arrange for someone to accompany you home afterwards, as you may have some swelling or bruising.

During the Procedure

You lie on a procedure table. The leg is cleaned with antiseptic. A very fine needle puncture in the skin allows the laser fibre to be introduced into the vein under ultrasound guidance. Tumescent local anaesthesia is injected — this is the most uncomfortable part of the procedure, similar to dental injections. Once the leg is numb, the laser treatment begins. You may feel mild warmth or pressure but no significant pain. The entire procedure typically takes 30–60 minutes depending on the number of veins treated.

After the Procedure

A compression stocking is applied immediately. You walk out of the procedure room. You should walk for at least 30 minutes before leaving the hospital. Most patients drive home themselves or take a taxi. You may take over-the-counter pain relief (paracetamol or ibuprofen) for any post-procedure discomfort.

✅ Benefits of EVLT

  • No general anaesthesia required
  • No surgical incision — just a needle puncture
  • Walk-in, walk-out day procedure
  • Return to office work within 1–2 days
  • Excellent long-term results (over 90% closure rate at 5 years)
  • Significantly lower complication rate vs traditional surgery
  • Can treat both legs in a single session if needed

🏥 Recovery Timeline

Days 1–3

Bruising and firmness along the treated vein are normal. Mild aching or tightness is expected — managed with simple analgesics. Walk regularly — at least 30 minutes twice daily. Wear the compression stocking continuously.

Week 1–2

Return to sedentary or office work typically within 1–2 days. Continue wearing the compression stocking. Avoid heavy lifting or strenuous exercise. A follow-up ultrasound at 1 week confirms the vein is successfully closed.

Weeks 3–4

The compression stocking is discontinued. Return to full exercise, including gym work and running. The treated vein gradually fades and disappears over several weeks.

🙋🏻‍♂️ Frequently Asked Questions

Is laser treatment for varicose veins permanent?

The treated vein is permanently closed. However, new varicose veins can develop from other segments of the venous system over time. The laser does not prevent future veins from developing in untreated segments.

Will I need more than one session?

In most cases, a single session treats the main refluxing trunk vein. If there are multiple refluxing segments, additional sessions may be recommended. This is discussed in detail at your pre-operative consultation.

Is EVLT covered by health insurance in India?

Yes — in most cases, EVLT for symptomatic varicose veins is covered under major health insurance policies in India, including government schemes. Your treating team can assist with pre-authorisation documentation.

🔑 Key Insights

EVLT represents the gold standard in varicose vein treatment — minimal discomfort, rapid recovery, and excellent long-term results without the need for general anaesthesia or surgery.
