Varicose veins affect a large proportion of the adult population, and genetics is the single biggest risk factor. If your parents had varicose veins, there is a significant chance you will too. However, genetics is not destiny. The way you live, work, and care for your legs can meaningfully influence whether varicose veins develop, how quickly they progress, and how severe they become.

🚶♂️ Move More — Every Day
The calf muscle is the most powerful pump in the venous return system. Every time you walk or contract your calf, blood is squeezed upward against gravity back toward the heart. Prolonged sitting or standing with still legs allows blood to pool in the superficial veins, increasing pressure on the vein walls and valves over time.
The most effective movement-based preventions are simple. Walking for at least 30 minutes daily keeps the calf pump active. If you have a desk job, stand up and walk for 2–5 minutes every 30–60 minutes. If you stand for long periods at work (teachers, surgeons, retail staff), shift your weight regularly and walk in place when possible. Calf raise exercises — rising on the balls of your feet repeatedly — can be done discreetly anywhere and are excellent for venous return.
⚖️ Maintain a Healthy Weight
Excess body weight increases intra-abdominal pressure, which directly impairs venous blood flow from the legs back to the heart. It also increases the load on vein walls and valves in the lower limbs. Achieving and maintaining a healthy weight through a balanced diet and regular exercise is one of the most effective long-term protective measures against varicose veins.
Even a modest weight reduction of 5–10% in overweight individuals can significantly reduce venous pressure and symptom burden in those who already have varicose veins.
🧦 Compression Stockings — Who Should Wear Them?
Graduated compression stockings apply the most pressure at the ankle, gradually decreasing up the leg. This gradient mimics and supports the natural venous return mechanism. Compression stockings do not cure or reverse varicose veins, but they effectively control symptoms, slow progression, and significantly reduce the risk of complications such as venous ulcers and superficial thrombophlebitis.
Compression stockings are particularly recommended for people at high risk who are not yet ready for treatment, those who are pregnant (specialist maternity compression hosiery), individuals who stand or sit for long periods at work, people who have already had varicose vein treatment to maintain results, and frequent long-haul travellers.
A vascular surgeon or trained nurse can advise on the appropriate compression class (Class 1, 2, or 3) for your individual situation.
🛍️ Elevate Your Legs
Elevating the legs above the level of the heart when resting allows gravity to assist venous drainage, reducing the pressure and pooling in leg veins. This is particularly valuable at the end of a long day on your feet. Lie on your back with your legs resting on two pillows, or raise the foot end of your bed by a few centimetres. Even 15–20 minutes of leg elevation in the evening can significantly relieve symptoms and reduce overnight swelling.

Low-impact aerobic exercise that engages the calf muscles is excellent for venous health. Swimming is considered one of the best exercises for varicose veins — the horizontal body position removes gravitational pressure on leg veins while calf contractions actively pump venous blood. Cycling and walking are similarly beneficial. Running is also fine for most patients and does not worsen varicose veins, despite common misconceptions.
Heavy weightlifting involving Valsalva manoeuvres (breath-holding during heavy exertion) can temporarily increase venous pressure and may aggravate symptoms. This does not mean avoiding the gym — it means focusing on technique, avoiding excessive breath-holding, and wearing compression during training if you have symptomatic varicose veins.
👟 Footwear Matters
High heels reduce the effectiveness of the calf muscle pump because they limit ankle movement and range of motion. Low or flat shoes, particularly those that allow a full heel-to-toe walking gait, are better for venous health. This does not mean never wearing heels — but alternating with comfortable, flat shoes throughout the day can help. Supportive footwear that encourages natural walking also reduces leg fatigue associated with varicose veins.
🤥 Avoid Excessive Heat
Heat causes veins to dilate. Hot baths, prolonged sun exposure on the legs, saunas, and hot yoga can temporarily worsen varicose vein symptoms — particularly aching, heaviness, and swelling. This does not mean these activities are strictly forbidden, but if you notice your symptoms worsen significantly after heat exposure, minimising them is a simple and effective strategy. Cold showers or ending a warm shower with a cool rinse on the legs can help constrict the veins and reduce discomfort.
🤱 Pregnancy and Varicose Veins
Pregnancy is one of the most common triggers for varicose veins. The combination of increased blood volume, hormonal changes that relax vein walls, and the growing uterus compressing pelvic veins all contribute. Women who are pregnant or planning pregnancy and have a family history of varicose veins should be particularly proactive.
Wearing graduated maternity compression hosiery from early in pregnancy can significantly reduce the development and severity of varicose veins. Staying active, maintaining a healthy pregnancy weight, elevating legs when resting, and sleeping on the left side (which reduces compression of the inferior vena cava) all help. After delivery, varicose veins that developed during pregnancy may partially or fully resolve — typically assessed at 3–6 months post-partum before any treatment is planned.
🚱 Avoid Tight Clothing Around the Waist and Groin
Tight clothing around the waist, upper thighs, or groin can restrict venous return from the legs by compressing the venous outflow pathways. Loose-fitting clothing, particularly around the waistband and thigh, supports better venous drainage. This is a minor contributing factor but relevant for people who are already symptomatic or at high risk.
🔍 Know Your Family History and Get Checked Early
If one or both parents had varicose veins, your risk is substantially elevated. Many people live with significant venous reflux for years before obvious varicose veins appear — during which time valve damage and venous pressure changes are already progressing. An early assessment by a vascular surgeon, including a Duplex ultrasound if indicated, can identify subclinical reflux and allow preventive strategies to be targeted appropriately.
