1. Progressive Skin Changes
Chronic venous hypertension causes a cascade of skin changes: haemosiderin pigmentation (brownish discolouration), venous eczema (inflamed itchy skin), and lipodermatosclerosis (fibrosis of the skin and fat). These changes are irreversible once established and indicate that ulceration is imminent.
2. Venous Leg Ulcers
The most serious skin complication — open, painful wounds above the inner ankle that are very difficult to heal and have a 70% recurrence rate without treatment of the underlying reflux. Venous ulcers cause significant pain, reduced mobility, and severely impact quality of life.
3. Superficial Thrombophlebitis

Large varicosities close to the skin surface can rupture with minimal or no trauma, causing profuse bleeding. This is particularly dangerous in elderly patients on blood thinners. It is a true medical emergency.
6. Phlebitis and Infection
Chronically dilated varicose veins are more susceptible to inflammation and, in some cases, secondary bacterial infection through skin breakdown. This can progress to cellulitis requiring hospitalisation.
