If you or someone you care for has been diagnosed with chronic kidney disease (CKD), you have likely heard about dialysis — and perhaps about the AV fistula that makes haemodialysis possible. What most patients and even many general physicians do not know is when the vascular surgeon needs to enter the picture.

The answer, according to international guidelines — and from my own experience operating on hundreds of dialysis access patients — is: much earlier than most people think.

📋 What the Guidelines Say

The KDOQI guidelines recommend that CKD patients be referred to a vascular surgeon for access planning when eGFR falls to 20–25 mL/min/1.73m² (roughly CKD Stage 4). Why so early? Because an AV fistula requires 6-8 weeks to mature after surgery before it can be used.

⏳ The Timeline That Matters

  • eGFR 20–25: Refer to vascular surgeon for assessment, vein mapping, and access planning
  • eGFR 15–20: Fistula surgery ideally performed
  • eGFR 10–15: Dialysis typically initiated — the fistula should be ready
  • eGFR <10 with no access: Emergency catheter placement required — entirely avoidable

🧬 Why Vein Mapping Is Crucial Before Surgery

The first thing I do when a CKD patient is referred is a vein mapping ultrasound. This assesses the calibre and patency of the cephalic and basilic veins, the quality of the radial and brachial arteries, evidence of central vein stenosis, and the optimal site for fistula creation.

⚠️ What Damages the Veins Before Referral

  • IV cannulas in the forearm — every puncture creates a scar
  • Blood draws from antecubital veins — scars the primary fistula vein
  • PICC lines — can eliminate an entire arm for fistula creation
  • Central venous catheters — can cause central vein stenosis

🟢 Who Should Refer — and When

The referral should come from the treating nephrologist when eGFR approaches 20–25. If you are a CKD patient and your nephrologist has not yet mentioned vascular access planning, it is entirely reasonable to ask: “Should I be seeing a vascular surgeon to plan my dialysis access?”

My eGFR is 22 and my nephrologist hasn’t mentioned a vascular surgeon. Should I ask?

Yes, absolutely. At eGFR 22, you are in the referral window. If haemodialysis is likely in your future, getting vein mapping and surgical planning done now is the right move.

How long does fistula surgery take and what is recovery like?

AV fistula surgery is usually a 45–60 minute procedure under local or regional anaesthesia, often done as a day case. The fistula then takes 6–8 weeks to mature before it can be used for dialysis.
