The good news about diagnosing DVT is that it doesn’t require anything invasive or painful. In most cases, one scan and possibly one blood test give a definitive answer within the hour.
🔍 Doppler Ultrasound — The First and Usually Only Test
A venous Doppler ultrasound uses sound waves to visualise blood flow and directly see whether a vein is compressible (normal) or blocked by a clot (DVT). It’s painless, takes about 15–20 minutes, and needs no preparation or injections. This is the test I order for almost every patient with suspected DVT.
🧪 D-Dimer Blood Test
D-dimer measures a breakdown product of blood clots. A negative D-dimer in a low-risk patient makes DVT very unlikely and can sometimes avoid the need for a scan. However, a positive result doesn’t confirm DVT — many things raise D-dimer (infection, surgery, pregnancy, cancer) — so it’s used to rule out, not rule in, the diagnosis.
📋 Clinical Risk Scoring
Before any test, I assess your clinical probability using a scoring system (the Wells Score) that weighs your symptoms, risk factors, and exam findings. This guides which test to do first and how urgently — a high-probability patient goes straight to ultrasound, while a low-probability patient may start with D-dimer.
🫁 When Additional Tests Are Needed
If pulmonary embolism is suspected (breathlessness, chest pain), a CT pulmonary angiogram is used to directly visualise the lung arteries. For patients with an unprovoked DVT and no clear risk factor, further blood tests for clotting disorders — or age-appropriate cancer screening — may be recommended.
💬 My Clinical Perspective
Patients are often relieved to learn the diagnostic process is this straightforward — no needles into the vein, no dye, no overnight stay required just to get an answer. What matters most is not delaying that first ultrasound.
❓ Frequently Asked Questions
Is the DVT ultrasound painful?
No. It’s a painless external scan using a handheld probe on the leg, similar to a pregnancy ultrasound. No needles or injections are involved.
Can a blood test alone diagnose DVT?
A D-dimer blood test can help rule DVT out in low-risk patients but cannot confirm the diagnosis on its own, since many conditions raise D-dimer levels. Confirmation requires ultrasound imaging.
How long does it take to get DVT test results?
Doppler ultrasound results are usually available immediately, as the scan is reviewed in real time. D-dimer blood results typically return within 1–2 hours.
