Stroke is the second leading cause of death and the leading cause of serious long-term disability worldwide. A significant proportion of strokes — roughly 20–30% — are caused by disease in the carotid arteries, the large arteries on either side of your neck that supply blood to the brain. These strokes are largely preventable with timely vascular intervention.
🦠 What Is Carotid Artery Disease?
The carotid arteries are the main arteries supplying blood to the brain. Carotid artery disease occurs when atherosclerotic plaque builds up inside these arteries, narrowing the lumen and creating two dangers: reduced blood flow to the brain, and embolism — where fragments of plaque or clot break off and travel up to the brain, causing a stroke or transient ischaemic attack (TIA).
🚨 Symptoms and Warning Signs
Most carotid artery disease is asymptomatic — patients feel nothing. The first warning may be a TIA (transient ischaemic attack), sometimes called a “mini-stroke”: a brief episode of neurological symptoms (weakness on one side, slurred speech, vision loss in one eye) that resolves within 24 hours, usually within minutes.
A TIA is a medical emergency and a major warning sign that a full stroke may be imminent. Do not ignore it or wait until the next morning to seek help.
Other symptoms may include a bruit — an abnormal “whooshing” sound heard with a stethoscope over the carotid artery — though many significant stenoses have no bruit.
🔬 Diagnosis
- Carotid Duplex Ultrasound — The first-line investigation. Non-invasive, accurate, and widely available. Measures the degree of narrowing and assesses plaque characteristics.
- CT Angiography — Provides precise anatomical detail of the carotid bifurcation and intracranial vessels. Essential for surgical planning.
- MR Angiography — Alternative for patients avoiding radiation or contrast dye.
💉 Treatment Options
Medical Management
All patients with carotid artery disease require aggressive medical management: antiplatelet therapy (aspirin, clopidogrel), high-dose statins, blood pressure control, diabetes management, and smoking cessation. For mild to moderate stenosis (<50%), medical management alone is often sufficient.
Carotid Endarterectomy (CEA)
CEA is the gold standard surgical treatment for significant carotid stenosis (typically 70–99% narrowing with symptoms, or >60% in some asymptomatic high-risk patients). The surgeon makes a small incision in the neck, temporarily clamps the artery, and removes the plaque from inside. The artery is then repaired with a patch. Most patients go home within 2–3 days.
CEA has an excellent track record of stroke prevention — multiple large trials have confirmed it reduces the risk of subsequent stroke by over 50% compared to medical therapy alone in symptomatic patients with significant stenosis.
Carotid Artery Stenting (CAS)
CAS is a catheter-based alternative to CEA, particularly suitable for patients who are high surgical risk due to medical comorbidities, or who have anatomically challenging carotid disease. A stent is deployed inside the narrowed artery to prop it open, with a distal protection device used to catch any emboli released during the procedure.
❓ Frequently Asked Questions
My ultrasound shows 60% carotid stenosis but I have no symptoms. Should I have surgery?
This is a nuanced decision that requires careful individual assessment. In general, for asymptomatic stenosis, surgery is considered only at higher degrees of stenosis in patients who are young, fit, and at a low operative risk centre. Most asymptomatic patients are managed medically with surveillance. Discuss your specific situation with a vascular surgeon.
I had a TIA last week. How urgently do I need to see a vascular surgeon?
Urgently — ideally within 48–72 hours. The risk of a full stroke is highest in the days immediately following a TIA. If carotid stenosis is the cause, early CEA dramatically reduces this risk. Do not wait for an elective appointment.
💡 Key Insight: A TIA is the brain’s fire alarm. If the fire alarm goes off, you investigate immediately — you don’t wait to see if the building actually catches fire. A carotid duplex ultrasound after a TIA is not optional. It is urgent.
