When arteries in the leg are so badly blocked that angioplasty is not feasible — or has failed — bypass surgery is the gold standard. The procedure creates a new route for blood to flow around the blockage, using either the patient’s own vein (the best option) or a synthetic graft. Here is a complete explanation of what bypass surgery involves, who needs it, and what to expect.

🦠 When Is Bypass Surgery Needed?

  • Long-segment occlusions of the superficial femoral or popliteal artery not suitable for angioplasty
  • Critical limb ischaemia with rest pain or tissue loss where endovascular options have failed
  • Diabetic foot with distal tibial artery disease requiring below-knee bypass
  • Failed previous angioplasty or in-stent restenosis

💉 Types of Leg Bypass

Femoral-Popliteal Bypass

The most common bypass for PAD. Blood is rerouted from the common femoral artery (groin) to the popliteal artery (behind the knee) or below the knee. The great saphenous vein from the patient’s own leg is the preferred conduit — it performs significantly better than synthetic grafts in terms of long-term patency.

Femoral-Tibial Bypass

Used when the blockage extends to the tibial arteries — the small arteries below the knee supplying the foot. This is the bypass most commonly used for diabetic foot salvage. A vein bypass to a tibial or peroneal artery below the knee can restore enough blood flow to heal a foot wound and avoid amputation.

Aorto-Bifemoral Bypass

For blockages involving the aorta and both iliac arteries, an aorto-bifemoral bypass connects the aorta to both femoral arteries using a Y-shaped synthetic graft. This is a major operation requiring general anaesthesia and a hospital stay of 5–7 days, but provides excellent long-term results.

🏥 What to Expect

Bypass surgery is performed under general or spinal anaesthesia. Operating time ranges from 2–4 hours depending on the complexity and number of anastomoses. Hospital stay is typically 4–7 days. Full recovery to normal activity takes 4–6 weeks.

Post-operatively, patients are prescribed antiplatelet therapy (aspirin or clopidogrel) and undergo regular duplex ultrasound surveillance of the graft at 6 weeks, 3 months, 6 months, and annually thereafter — because early detection of graft stenosis allows salvage before it occludes.

❓ Frequently Asked Questions

Will the surgeon use my own vein or a plastic tube?

Your own vein (great saphenous vein) is always the first choice. Vein bypasses have superior patency rates compared to synthetic grafts, especially in below-knee bypasses. If your veins are not suitable (too small, already used, or varicosed), a synthetic graft (PTFE or Dacron) is used, usually with some modifications to improve results.

How long does a bypass graft last?

A well-constructed vein bypass has a 5-year primary patency of around 60–70% for above-knee bypasses and 50–60% for below-knee bypasses. With regular surveillance and prompt treatment of any graft stenosis, assisted patency rates are significantly higher.

💡 Key Insight: The saphenous vein is literally the most valuable vein in your body if you ever need cardiac or vascular bypass surgery. Patients with varicose veins should always discuss with their surgeon before having the vein stripped or ablated — particularly if they have risk factors for PAD or coronary artery disease.
