Most people think varicose veins are obvious — large, rope-like bulges snaking down the leg. But in my vascular surgery practice in Hyderabad, I regularly diagnose significant venous reflux in patients who have only mild symptoms. 🔵 Early and Mild Symptoms of varicose veins

Leg Heaviness and Fatigue

One of the earliest symptoms of venous reflux is a persistent sense of heaviness or fatigue in the legs, particularly after standing for extended periods. Many patients attribute this to normal tiredness for years before a diagnosis is made.

Aching or Throbbing Pain

A dull, aching, or throbbing pain along the course of a vein — typically the inner calf, inner thigh, or behind the knee — is highly characteristic of venous disease. This pain worsens as the day progresses and improves with leg elevation.

Itching and Burning

Itching or a burning sensation along a vein, particularly around the ankle and lower calf, is often the first skin-related symptom. It is caused by chronic low-grade inflammation from venous hypertension.

Night Cramps

Nocturnal calf or thigh cramps are frequently associated with venous reflux. Recurrent night cramps alongside other venous symptoms should prompt investigation.

Restless Leg Sensation

An uncomfortable urge to move the legs at rest in the evening can be a manifestation of venous insufficiency. Treating the underlying reflux often significantly improves this symptom.

Thread Veins and Spider Veins

Fine red, purple, or blue thread veins visible just beneath the skin surface are often the first visible sign. Their presence in a symptomatic patient warrants a Duplex ultrasound to check for underlying reflux.

Visible Bulging Veins

Dilated, tortuous, rope-like veins visible beneath the skin — typically along the inner calf, thigh, or behind the knee. More prominent after prolonged standing and less noticeable when lying down.

Ankle Swelling

Fluid accumulation around the ankles and feet by evening indicates that venous pressure has risen to the point where fluid leaks into surrounding tissues. This should not be dismissed without investigation.

  • Skin hardening (lipodermatosclerosis)
  • Eczema on the lower leg (venous eczema)

Can I have varicose veins without any visible veins?

Yes. Significant venous reflux can exist with minimal visible abnormality. Symptoms such as leg heaviness, aching, and swelling may be the only manifestation.

In most patients, untreated varicose veins worsen progressively. Skin changes and ultimately ulceration represent the end-stage of a disease that begins with simple reflux.
